Telehealth Grant Funding Available through the Colorado Health Foundation

The Colorado Health Foundation has announced a new funding opportunity within its healthcare outcome area. Its focus is to ensure all Coloradans achieve health with support from a network of primary healthcare and community services.

Telehealth Funding Opportunity
As part of the foundation’s strategy to strengthen the delivery of comprehensive, person-centered primary care, this initiative will build capacity in primary care clinics for the implementation of telehealth services. Funding will support up to $100,000 for costs associated with the purchase of telehealth equipment and/or the employment of workflow or billing consultants. Private practices also may apply for low-interest loans.
The foundation seeks applicants who focus on:
  • teleconferencing between providers and patients,
  • telemonitoring to increase regular contact with care providers and monitor conditions, and
  • other innovative telehealth programs that increase access to comprehensive, coordinated primary care services through healthcare-related technology.
Applicants must provide primary care services in Colorado to a significant population of uninsured and publicly insured individuals. Preference will be given to clinics providing integrated (physical, behavioral or oral) care services. To be considered for funding, applicants must meet specific criteria.
