The CDA is surveying ALL Colorado dentists (Medicaid and non-Medicaid providers, those who see children and those who don’t) and dental hygienists. The survey results will help the CDA better understand dentists’ experiences with the Take 5/pay-for-performance program to gather data on what influenced them to sign up or not sign up as Medicaid providers when the program was expanded in 2014.
The survey takes less than 10 minutes to complete. This data will be invaluable in shaping future policy in Colorado.
