CDA Discontinues Laboratory Pad Program

The CDA has discontinued its laboratory pad program due to declining utilization by dental offices. We apologize for any inconvenience this may cause. Please check with your dental software provider regarding lab work orders. Many if not all the major providers include this option within their programs.
We also encourage you to check with your dental lab – they may have work order forms that you can access and use.
 

The Colorado Dental Board requires dental offices to retain a copy of each work order for two years. Work orders, at a minimum, should include:
A. Name of laboratory.
B. Name of dentist.
C. Address of dentist.
D License number of dentist.
E. Patient name or l.D. number.
F. Instructions to laboratory.
1. Include adequate space for instructions or directions.
2. Date of try in or delivery.
G. Personal signature of the authorizing dentist written in ink or provided electronically, manually entered by the dentist for each order (the use of rubber stamped, pre-printed or a pre-signed signature on work orders is not acceptable).
H. Date of directions.
