On Feb. 4, 2011, dentists across the state volunteered for Give Kids a Smile Day, a national access-to-care day for underserved children. Thank you to the hundreds of Colorado volunteers who made this year’s program a success! Your volunteer time and support of this annual program is greatly appreciated.
The CDA is busy compiling the reports from this year’s event, but we need your help. If you have NOT submitted your GKAS reporting forms, please take just a few minutes to fax or mail them to the CDA.
This is the only method the CDA has for recording the volunteer contributions for the program. These numbers are shared with legislators and the media.
If you treated patients, please fill out a GKAS Superbill for each patient you saw and fax/mail them to the CDA. Click here to download a GKAS Superbill.
